What is Direct Primary Care?

Direct Primary Care (DPC) is a modern healthcare model where patients pay a flat monthly membership fee directly to their doctor’s office for routine primary care.

Think of it like a gym membership. You pay the same monthly fee each month in exchange for greater access to your physician, visits for both sick and wellness care and potentially access to wholesale pricing for medications, labs and radiology.

The Two-Hour Wait in Temple

Javier is a local business owner in Temple. He was fed up with the assembly line feel of his doctor’s office. He’d wait two weeks for an appointment, sit in the waiting room for an hour, and finally get ten minutes with a doctor who spent most of the time typing into a computer.

When Javier switched to a direct primary care practice, everything changed. Last month, when he felt a sinus infection coming on, he texted his doctor directly. He was seen that afternoon, spent 45 minutes talking through his health goals, and walked out with a prescription that cost him $4. No insurance paperwork, no facility fees, and no surprise bills in the mail three weeks later.

The DPC Model: How It Works

In a DPC arrangement, the relationship is strictly between the doctor and the patient.

  • The Membership Fee: Usually ranging in price depending on the doctor and the location, but generally less than $100 per month. This membership fee covers all office visits, wellness exams, and potentially minor procedures.
  • Wholesale Pricing: Often, DPC doctors are able to get cash pricing rates for labs and radiology exams. Some DPC doctors are even able to buy prescription medications at wholesale prices and pass the cost savings on to you. Currently, however, Texas physicians are not allowed to dispense medications from their office. But they may be able to help you navigate how to get prescription medications for less.
  • Enhanced Access: Most DPC doctors limit their patient panel size to a fraction of a traditional practice. We are talking hundreds of patients on the panel instead of thousands. This often means that they are available for same-day appointments and more direct access (texting, phone, email) than physicians who practice in a traditional setting.

DPC is NOT Insurance (But It Pairs Well With It)

It is vital to understand that a DPC membership is not a substitute for comprehensive health insurance. It doesn't cover hospital stays, surgeries, or catastrophic events.

For many Texans, the smartest option is a two-part strategy:

  1. DPC Membership: For your day-to-day healthcare, chronic disease management, and urgent care needs.
  2. High-Deductible PPO or HMO plan, Catastrophic Health Coverage or Health Sharing Plans: For the big stuff like a car accident or a cancer diagnosis.

This combination offers better day to day care while accounting for coverage for catastrophic health events.

Why Texas Employers Are Flocking to DPC

Small business owners in Texas are increasingly using DPC as a cornerstone of their employee benefits. By pairing a DPC membership with a lower-cost, high-deductible plan, employers can give their team concierge-level access to a doctor. This reduces absenteeism, improves chronic care management, and ensures that employees aren't avoiding the doctor because they're afraid of an expensive co-pay.

Understanding the Costs

  • DPC fees are separate: Membership fees do not get applied toward your health insurance deductible.
  • Dual payment: You maintain both a health plan and a DPC membership. A combination that often delivers the highest level care for chronic condition management.
  • Lower total spend: Superior primary care access through a DPC can reduce your overall out-of-pocket medical expenses.
  • Strategic pairing: DPC is most effective when combined with a High Deductible Health Plan (HDHP) or catastrophic coverage.

Restore the Doctor-Patient Relationship

The DPC movement is all about taking the system out of the exam room. It’s about returning to a time when your doctor actually knew your name and your history, rather than just your insurance ID number.
